Right selection of oral anticoagulation for stroke prevention in atrial fibrillation

چکیده
Atrial fibrillation (AF) is considered as one of the most common cardiac arrhythmias worldwide and always associated with a significantly increased risk stroke thromboembolism. VKAs (Warfarin & Acenocoumarol) are highly effective in reducing patients AF, but bleeding issues disutility sparked development direct oral anticoagulants (DOACs) or NOACs (Novel anticoagulants). These drugs, offer convenience once twice daily dosing without need for laboratory monitoring coagulation activity (PT/ INR) routine dose adjustment. Various clinical trials proved their non-inferiority to warfarin (VKA) systemic embolism, each was markedly lower rates ICH than well-adjusted warfarin. Yet there no head-to-head comparative efficacy safety NOACs. In this review we try provide patient centric approach assist Indian physicians selecting right OAC therapy SPAF respect best possible evidence recommendations available worldwide.

Anticoagulation for stroke prevention in atrial fibrillation: is gender important?
The presence of atrial fibrillation (AF) confers a five-fold increased risk of stroke, a figure that may rise to as high as 17 times in the presence of structural heart disease, in particular mitral stenosis. It is estimated that 15% of all strokes may be directly attributable to AF, but of greater concern is that when patients with AF have a stroke, they have a much worse outcome. Being an inc...
متن کاملChoosing the right drug to fit the patient when selecting oral anticoagulation for stroke prevention in atrial fibrillation.
Atrial fibrillation (AF) is the most common cardiac arrhythmia worldwide and is a growing health problem that is associated with a significantly increased risk of stroke and thromboembolism. Oral anticoagulant (OAC) therapy reduces the risk of stroke and all-cause mortality in patients with AF. OAC therapy is commonly given as a well-controlled vitamin K antagonist (VKA; e.g. warfarin) and can ...
متن کاملAnother oral thrombin inhibitor for stroke prevention in atrial fibrillation?
Atrial fibrillation (AF), the most common arrhythmia, is associated with a five-fold increase in the risk of stroke (1). Furthermore, cardio-embolic strokes in AF patients tend to be severe, and are often fatal or associated with serious morbidity. Vitamin K antagonists, such as warfarin, produce about a 64% reduction in the risk of stroke in AF patients (2), but have numerous limitations that ...
متن کاملPrevalence of oral anticoagulation in atrial fibrillation
OBJECTIVES Atrial fibrillation is the most common sustained arrhythmia and is associated with poor outcomes, including stroke. The ability of anticoagulation therapy to reduce the risk of stroke has been well established; however, the prevalence of anticoagulation therapy use in the Public Health System is unknown. The aim of this study is to evaluate both the prevalence of anticoagulation ther...

2014; 111: 465-473. Note: The review process for this editorial was handled by Christian Weber, Editor in Chief. Clinical guidelines for the management of atrial fibrillation (AF) advocate individual stroke risk assessment with CHA2DS2-VASc to determine a patient’s suitability for oral anticoagulation (OAC), followed by formal appraisal of their bleeding risk associated with OAC, using the HAS-...
